One-Time Hospital-to-Home Care Services

New routines, regiments, and risks can be overwhelming for those who experience it for the first time. Thankfully, our staff members are experts in safely guiding clients on the next step of their healing journey.

When your loved one begins a transition of care, our team will meet with medical personnel, review discharge procedures, pick up prescriptions, and safely get your loved one home. Caregivers will then review the discharge plan with your family, answering any questions you may have.

Hospital-to-Home service includes:

  • An experienced, reliable caregiver who meets your loved one in the hospital room or discharge area
  • Transportation home with stops for picking up prescriptions or groceries
  • Help understanding and following discharge plans
  • A debriefing phone call to the family (or doctor or health coach)
  • Scheduling and attending follow up appointments
  • Meal preparation
  • Light housekeeping and laundry
  • Identify trip hazards and make home safety recommendations
  • Educate the client and family members on red flags or warning signs for chronic conditions
  • Bathing, grooming, and personal hygiene
