The In-Home Health Care Process
To begin the application process, the ComForCare Home Care Admission Service Agreement is completed, and then a registered nurse (RN) from our agency meets with the prospective client/family. During that first meeting the nurse writes a comprehensive assessment that outlines the care plan and specific client needs. A copy of the assessment stays in the client’s home and serves as a guide for the ComForCare caregivers. The RN provides case management by staying in touch with the client, the responsible party, and the caregivers. The RN makes regular home visits to the client to reassess needs and levels of care.
